In this role, you will receive a $12,000 SIGN-ON BONUS.

Responsibilities include:

• Oversee direct nursing care to patients on your 38 bed half long term care, half sub-acute unit.

Role Responsibilities:

• Complete case start-to-finish: DX, MDS, Care Planning, Case Management
• Lead and manage managed care caseloads while flexing to support LTC and Medicare MDS as needed
• Conduct timely and accurate assessments (5–6 per week typical in LTC, plus subacute/managed care)
• Collaborate with IDT to ensure compliance with federal/state regulations and QRP thresholds
• Support training initiatives (e.g., Section GG) and contribute to ongoing staff development

The Hospice Clinical Manager provides leadership for hospice clinical services, ensuring highquality care and alignment with the organization’s mission and standards. • Oversight of direct and indirect patient services provided by nursing and home health aide teams

Qualifications:

Hospice Chaplain Responsibilities:

• Performs spiritual assessment on all Hospice patients and families who desire spiritual support based on their religious, spiritual and cultural practices and beliefs.

• Acts as liaison to and resource for community clergy; coordinates with community clergy to arrange participation in spiritual rituals and pastoral support.

• Provides nondenominational spiritual counseling per patient and family, providing support through discussion of spiritual and existential issues which may include prayer, counsel and support.

• Responds to the spiritual, social and emotional needs of terminally ill patients and their families as it relates to their spiritual/existential needs.

• Functions effectively as part of the interdisciplinary team, attending and participating in staff and interdisciplinary team meetings.

• Documents in EMR and in writing of services provided to patient and family in a complete and timely manner.

• Provides bereavement follow-up care as needed in conjunction with bereavement coordinators.

• May assist with bereavement support groups and provides referrals to family for community grief support groups or individual counseling as needed.

• Actively participates in Quality Assessment and Performance Improvement (QAPI) activities.

• Represents the organization in a positive and professional manner in the community, communicating the mission, ethics and goals of our organization.

Hospice Chaplain Professional Requirements:

• Graduate of an accredited seminary or school of theology (BD, MDiv, Master of Theology or equivalent theological degree).

• Four units of Clinical Pastoral Education preferred.

• Experience working with death and dying, individuals/family/caregivers preferred.

• Excellent interpersonal and communication skills.

• Ability to work as a member of an interdisciplinary group.

• Ability to work with socio-culturally diverse patients/families of all faiths and accept different lifestyles, cultures, beliefs, and values.

Pay: $31.00 – $47.50 per hour (based on experience)
Sign-On Bonus: $12,000.00 for Full-Time (pro-rated for part-time)

About the Role

Broad Reach Healthcare is looking for a dedicated and compassionate Nurse (RN or LPN) to join our team. In this role, you’ll provide consistent, high-quality nursing care to residents who call our community home. You’ll build meaningful relationships with residents and families while ensuring comfort, safety, and dignity each day.

Responsibilities

• Provide daily nursing care and medication administration according to each resident’s care plan.
• Observe, record, and report changes in resident conditions to the clinical team.
• Supervise and support CNAs and other direct care staff.
• Ensure a safe, comfortable, and respectful environment for residents.
• Collaborate with interdisciplinary team members to promote resident wellness.
• Follow all facility, state, and federal regulations for long-term care.

Qualifications

• Active Massachusetts RN or LPN license (required).
• Strong clinical assessment and documentation skills.
• Compassionate, dependable, and resident-focused.
• Ability to work independently and as part of a caring team.
