The Hospice Clinical Manager provides leadership for hospice clinical services, ensuring highquality care and alignment with the organization’s mission and standards. Duties & Responsibilities:

• Oversight of direct and indirect patient services provided by nursing and home health aide teams

• Provides clinical oversight, mentoring, and guidance to interdisciplinary team members

• Assists in development and maintenance of clinical policies and procedures that comply with federal and state regulations and CHAP standards.

• Ensures compliance with healthcare regulations and governance standards

• Ensure compliance with the performance improvement program design by QAPI team.

• Communicate effectively with families, caregivers, and providers regarding goals of care and clinical updates

Qualifications:

• Licensed Massachusetts Registered Nurse - required

• Demonstrated ability to develop and maintain relationships in the communities served as well as within a company.

• Skilled at establishing/maintaining working relationships with key staff, marketing contacts, and clinical staff to ensure thorough understanding of Hospice Care Mission and services.

• Experience in analyzing processes, improvement planning, supporting change, and effectively managing priorities.

Your responsibilities will include conducting therapeutic activities, providing hands-on assistance, and documenting progress towards goals. In this unique model. you will be embedded into an interdisciplinary team dedicated to a specific pod of post-acute rehab patients.  It’s an extraordinary professional opportunity!

Responsibilities:

• Collaborate with occupational therapists to develop and implement individualized treatment plans.
• Conduct therapeutic exercises and activities to improve patients’ motor skills, strength, and coordination.
• Assist clients with activities of daily living, adaptive techniques, and use of assistive devices.
• Provide emotional support and encouragement to clients and their families.
• Maintain accurate and up-to-date documentation of treatment sessions and progress reports.

Qualifications:

• Associate’s degree from an accredited Occupational Therapy Assistant program.
• Current certification as a Certified Occupational Therapy Assistant (COTA) issued by the National Board for Certification in Occupational Therapy (NBCOT).
• State licensure or eligibility for licensure in Massachusetts.
• Strong communication and interpersonal skills.
• Compassionate and patient-centered approach to care.
• Ability to work effectively in a team environment.
